چکیده

Randomization plays a fundamental role in clinical trials. While many modern clinical trials employ restricted, stratifiedor covariate-adaptive randomization designs that pursue balance in treatment assignments and balance across important covariates, some clinical trials call for response-adaptive or covariate-adjusted response-adaptive (CARA) randomization designs to address multiple experimental objectives primarilyrelatedtostatisticalefficiencyandethicalconsiderations.Inthis paper,weelicitkeyprinciplesofthewell-conductedrandomizedclinical trial and explore the role of randomization and other important design tools in achieving valid and credible results. We give special attention to response-adaptive and CARA randomization designs, which have afirm theoretical basis, but aremore complex andmore vulnerable to operational biases than traditional randomization designs. We conclude that modern advances in information technology, rigorous planning, and adherence to the keyprinciples of thewell-conducted clinical trial shouldenablesuccessfulimplementationofresponse-adaptiveandCARA randomization designs in the near future.

تصادفی سازی در مطالعات کارآزمایی بالینی: از تئوری تا عمل

Background & Aim: Randomized clinical trials (RCTs) are studies which are able to provide the most valid evidence to compare various interventions in health research. Biases can affect the quality of research and ultimately make the results of a study invalid. One of the most important biases is selection bias. The best way to reduce selection bias is the use of random allocation. The aim of th...

Influence of selection bias on the test decision. A simulation study.

BACKGROUND Selection bias arises in clinical trials by reason of selective assignment of patients to treatment groups. Even in randomized clinical trials with allocation concealment this phenomenon can occur if future assignments can be predicted due to knowledge of former allocations. OBJECTIVES Considering unmasked randomized clinical trials with allocation concealment the impact of selecti...

Introduction to a generalized method for adaptive randomization in trials

BACKGROUND Ideally clinical trials should use some form of randomization for allocating participants to the treatment groups under trial. As an integral part of the process of assessing the effectiveness of these treatment groups, randomization performed well can reduce, if not eliminate, some forms of bias that can be evident in non-randomized trials. Given the vast set of possible randomizati...

Eliminating bias in randomized controlled trials: importance of allocation concealment and masking.

Randomization in randomized controlled trials involves more than generation of a random sequence by which to assign subjects. For randomization to be successfully implemented, the randomization sequence must be adequately protected (concealed) so that investigators, involved health care providers, and subjects are not aware of the upcoming assignment. The absence of adequate allocation concealm...
